Your blood type could potentially hold clues to your risk of experiencing an early stroke, according to recent findings.
A study conducted by experts at a University of Maryland (UMD), U.S. has shed light on the intriguing connection between blood types and the likelihood of suffering an ischemic stroke before the age of 60. Ischemic strokes, which account for a substantial majority of all strokes, occur when blood flow to the brain is obstructed, usually by a clot. The study reveals that individuals with certain blood types may be predisposed to these life-threatening events, while others might have a reduced risk.
To arrive at their conclusions, researchers analysed genetic data from nearly 17,000 stroke patients and more than half a million healthy individuals who had never experienced a stroke. Participants in the study ranged in age from 18 to 59. Through this extensive examination, scientists uncovered a compelling association between early-onset strokes and the chromosome segment that determines blood type.
What stood out was the difference in stroke risk linked to blood types. Following adjustments for variables such as sex and lifestyle factors, the team discovered that individuals with blood type O—the most prevalent blood group—had a 12% lower risk of suffering an early ischemic stroke compared to those with other blood types.
On the other hand, people with blood type A—a group encompassing roughly 36% of the population in certain regions—were found to have a 16% increased risk of experiencing such strokes at a younger age.
While this information may raise concerns among those with blood type A, experts have cautioned against unnecessary alarm or immediate actions based solely on these findings. The precise mechanism behind this heightened risk remains unclear. Researchers speculate that the answer likely lies in factors related to blood clotting—such as platelet activity, the behaviour of cells lining blood vessels, and circulating proteins—all of which contribute to clot formation. Previous studies have pointed to a potential link between blood type A and an increased propensity for developing clots in the legs, known as deep vein thrombosis.

Researchers emphasised that additional studies are crucial to better understand why these variations exist and how they might influence stroke risk in younger adults. They also acknowledged limitations within their study, including a lack of diversity among participants. Expanding future research to include broader demographic and ethnic representation could yield more comprehensive insights.
The implications of these findings are significant, particularly given the rising incidence of strokes among younger adults worldwide. Traditionally associated with older populations, strokes are now increasingly affecting individuals aged 20 to 44. In fact, data indicates that the rate of ischemic strokes in this age group has surged substantially in recent decades.
Between 1993 and 2015, the prevalence among American adults in this demographic rose from 17 cases per 100,000 people to 28 cases per 100,000. This alarming trend highlights the need for targeted preventive measures. In Malaysia, we are no way better. By 2019, Malaysia recorded 47,911 annual stroke cases, with ischemic stroke accounting for 72% of young adult cases (ages 18–50).

Traditional stroke risk factors remain relevant. High blood pressure, elevated cholesterol levels, diabetes, smoking, and excessive alcohol consumption are well-documented contributors. These lifestyle-related risks are consistent across all blood types and remain critical areas for intervention.
However, the findings regarding blood type introduce an intriguing new dimension to stroke prevention efforts.
