Can Eating Tomatoes Daily Help Fatty Liver? What Early Study Really Found

Can eating tomatoes each day help reduce fat accumulation in the liver? A recent study featured in Nutrients suggests it might, providing a new twist in the ongoing search for dietary tools to combat fatty liver or more accurately known as metabolic dysfunction-associated liver disease (MASLD).

This common liver condition, affecting millions globally, is notorious for its links to obesity, high blood pressure, and type 2 diabetes. Managing it remains a top priority for clinicians and researchers, with diet and lifestyle modifications forming the cornerstone of current recommendations.

The Mediterranean diet, with its emphasis on plant-based foods and healthy fats, continues to attract attention for its potential in preventing and managing liver disease.

Tomatoes, a staple in this diet, are rich in antioxidants like lycopene and carotenoids, compounds long thought to harbour benefits for cardiovascular and liver health.

But do tomatoes truly offer protection against fat buildup in the liver? The evidence, while compelling, is still emerging.

In this exploratory randomised controlled trial, researchers enrolled 79 adults diagnosed with MASLD. The study excluded individuals with obesity—classified as a body mass index (BMI) above 30—as well as those with high alcohol intake. Participants were divided into two groups: one followed their usual diet but excluded tomatoes entirely, while the other consumed 200 grams of raw tomatoes and 50 grams of tomato sauce daily. All tomato products were sourced from the same provider to ensure consistency.

Over six weeks, the research team monitored changes in body composition, liver fat content (hepatic steatosis), liver stiffness, cholesterol levels, and other relevant markers.

Surprisingly, both groups experienced a reduction in liver fat. However, the decrease was more pronounced among those eating tomatoes. Other measures such as liver stiffness, lipid profiles, body weight, and liver enzyme levels remained largely unchanged between the groups.

What does this mean?

According to the study authors, their findings suggest that regular tomato consumption can reduce hepatic fat accumulation independently of significant weight loss or changes in abdominal fat.

This points to a possible direct effect of tomato-derived nutrients on the liver. Yet, caution is warranted. The intervention lasted just six weeks, a relatively short period in the context of chronic disease management. Longer studies are needed to determine whether these early benefits are sustained over time.

The research population also warrants scrutiny. Most participants were men under the age of 65, and all had a BMI below 30. Many real-world patients with MASLD are older or have obesity and multiple metabolic risk factors. This limits how widely the results can be applied.

Another caveat lies in the study design itself. Participants were required to change their eating patterns as part of a formal research trial—a factor known to influence behaviour beyond the specific foods being tested. Such dietary interventions often prompt individuals to pay closer attention to overall food intake and healthy habits, which could confound results. In addition, reliance on self-reported adherence opens the door to inaccuracies.

The combined use of raw tomatoes and tomato sauce also complicates interpretation. It remains unclear whether one form is more effective than the other or if both are needed for optimal benefit. Importantly, the researchers did not measure blood levels of lycopene or carotenoids during the trial; therefore, it is not possible to confirm whether these specific compounds drove the observed effects.

While imaging showed a reduction in liver fat among tomato eaters, no significant improvements were noted in other clinical outcomes such as insulin resistance, inflammatory markers, or measures of liver scarring (fibrosis).

As experts point out, improvements on scans alone do not necessarily translate into meaningful changes in disease progression or overall health. The study remains hypothesis-generating rather than definitive.

Despite these limitations, the results are intriguing and point towards a simple dietary tweak that could potentially help manage MASLD.

It adds to a growing body of evidence highlighting the value of plant-based diets rich in vegetables and fruits for metabolic health. Tomatoes themselves offer more than just potential liver benefits—they have been linked to improved cardiovascular health and may possess anti-cancer properties due to their antioxidant content.

Still, medical professionals stress that individual foods should not become the sole focus when tackling complex conditions like MASLD. Instead, they advocate for comprehensive dietary strategies tailored to each person’s needs and risk factors. For those living with MASLD or at risk of developing it, working closely with healthcare providers remains essential.

The broader context is equally important. As research generally highlight, sustained weight loss—particularly reductions of 10% or more—offers the strongest evidence for improving steatohepatitis and fibrosis in people with obesity-related liver disease.

While tomatoes can form part of a healthy Mediterranean-style eating pattern, they should not distract from proven therapies that deliver meaningful improvements in both liver and cardiometabolic outcomes.

Practical considerations must also be kept in mind. For individuals with gastroesophageal reflux disease (GERD), for example, tomatoes and tomato-based products can aggravate symptoms due to their acidity. Many processed tomato products are also calorie-dense or contain added sugars, factors that could worsen insulin resistance if consumed excessively.

For now, incorporating tomatoes within a balanced diet appears safe for most individuals and may confer additional health advantages beyond liver protection. As always, decisions about diet should be made collaboratively with healthcare professionals who understand each patient’s unique health profile.

Looking ahead, ongoing research will continue to unravel how specific foods interact with our bodies at a molecular level—and which combinations may offer the greatest promise for preventing or managing chronic diseases like MASLD.

In the meantime, the humble tomato earns another moment in the nutritional spotlight: colourful, versatile, and possibly good for your liver too.
