Aging gracefully is a concept many aspire to, but it comes with its challenges. One significant concern is the decline in balance and gait, which can lead to falls, a serious risk for the elderly. Recent research highlights an intriguing link: the ability to balance on one leg may be a strong indicator of neuromuscular aging. This revelation suggests that focusing on balance exercises throughout life is crucial for maintaining health in our later years.
Longevity has been on the rise globally, thanks to improvements in healthcare and living standards. The World Health Organisation notes that healthy life expectancy has climbed from 58.3 years in 2000 to 61.9 years in 2021. However, these additional years are not always accompanied by good health.
These changes often stem from deteriorating sensory systems like vision and hearing, alongside neuromuscular degeneration. Such alterations significantly increase the likelihood of falls, which is a leading cause of mortality among older adults. But what influences the rate of such decline? While chronological age offers one perspective, biological age provides a more nuanced view, taking into account genetics and environmental factors. Identifying who might face these neuromuscular challenges as they age could greatly aid in prevention and management.
A study spearheaded by researchers at Mayo Clinic proposes a simple yet effective test: balancing on one leg. This ability appears to be a reliable marker of neuromuscular aging for both genders. Balance, particularly in a unipedal stance, requires multiple sensory inputs, neuromuscular coordination, and sufficient muscle strength. As observed in this study, balance on one leg diminishes most rapidly among healthy individuals as they age.
The study involved 40 healthy participants aged 50 and above, split equally between those younger than 65 and those older. Researchers collected data on age, height, weight, activity levels, and limb dominance before conducting various assessments. These included measuring grip and knee strength on the dominant side, standing balance tests with eyes open and closed on both two legs and one leg, and gait analysis during level walking using an optical motion capture system. They also calculated the dynamic stability margin during these walking trials.
Interestingly, while men generally exhibited higher grip and knee strength than women, both genders experienced a similar decline rate of 3.7% per decade. However, no significant age-related differences were noted in gait or dynamic stability. Nonetheless, movement during bipedal standing balances increased notably with age — 6.3% per decade with eyes open and 10.4% per decade with eyes closed — underscoring the decline in balance with advancing years.
Furthermore, the duration an individual could stand on one leg decreased with age for both sexes. The decline was at a rate of 2.2 seconds per decade for the non-dominant side and 1.7 seconds per decade for the dominant side. These findings suggest that the neuromuscular sensory system deteriorates more quickly than muscular strength alone.
Expert in neuromuscular medicine points out that as we age, our bodies develop ‘wear and tear’ unevenly. Our bodies have built-in redundancies; thus, if one part doesn’t function optimally, others can compensate to maintain our abilities. However, when balancing on one leg, this compensation becomes challenging if there is weakness in one leg.
The study reinforces the vital role of balance training across all life stages. Maintaining balance is particularly important as individuals age to prevent falls that could result in fractures or other injuries. Exercises such as step-ups, sideways walking (ten steps each way), heel-to-toe walking in a line, and one-leg stands are simple yet effective ways to enhance balance at any age.
When starting these exercises, it’s prudent to have something stable nearby for support in case of balance loss. Other exercise forms like tai chi, yoga, and pilates also promote strength and improve balance.
Experts echo this sentiment, emphasising that a robust strengthening programme is essential for maintaining balance and overall health at any age. A physical therapist can conduct a comprehensive assessment for those feeling off-balance to identify subtle asymmetries in leg strength. They can also evaluate core strength and posture, which significantly impact leg function.
For instance, weak core muscles or long-standing posture problems can lead to asymmetric pressure on nerves from the back to the legs, causing one leg to be slightly weaker than the other. Strengthening the torso is crucial for optimising leg strength and balance.
This study sheds light on an often-overlooked aspect of fitness — balance — highlighting its importance alongside strength, flexibility, and endurance. As we navigate through life’s stages, incorporating balance exercises into our routine can ensure better health outcomes as we grow older.
While ageing brings inevitable changes to our bodies, understanding these changes and adopting preventive measures can significantly enhance our quality of life in later years. With growing evidence supporting the benefits of balance training, it’s never too early or too late to start incorporating these exercises into daily routines.
